If you want to become an Employee you must have:
- 18 years of age and older.
- Provide an Employment Application or Resume
- Download and fill out the Act 33/Child Abuse Clearance form and mail with $10 cashier check payable to Department of Public Welfare.
- Criminal Clearance Pennsylvania State Police - $10 on-line credit Pay and receive on line.
- FBI fingerprinted record check in accordance with Public Law and the Child Protective Services Law.
- www.pa.cogentid.com $36 on line fee and fingerprint Location the Chester County Intermediate Unit (CCIU) Boot Road.
- A Completed Form I-9, Employment Eligibility Verification
- Copies of identification documents that satisfy the I-9 requirements. (see Form I-9 info. above)
- W-4 Employee’s Withholding Allowance Certificate
- Valid PA Drivers License
- Current State motor vehicle registration
- Automobile insurance
Co-Op Provider.com will provide:
- CPR training if not certified already
- Protective Behavioral Training
- Food handling and nutrition
- Continuing certification training.
- Information Technology (IT) Training
- ISP based Training (An agreement to carry out the client’s ISP; Individual Support Plan responsibilities)
- Pennsylvania Department of Transportation Bureau of Driver Licensing Ten Year Driver Record - $5 on- line credit pay receive copy on line.
- Support Service Worker Agreement by (ODP) Pennsylvania Office of Developmental Program
